JOB TITLE: Family Nurse Practitioner (General Clinic Practice)

JOB DESCRIPTION: Serve as an Advanced Practice Family Nurse Practitioner for the Upland Hills Health (UHH)owned clinics. Clinic locations include: Spring Green, Highland, Montfort and at the main campus in Dodgeville. These are all outpatient clinics and the NP will work under the supervision of the designated Medical Director. Will provide care in the following clincial areas: primary health care, OB/GYN, weight loss clinic and specialized health services to individuals, families, groups, and communities on an outpatient and inpatient basis. Provides initial and ongoing assessment of patients’ and residents’ medical, physical, and psychosocial status to provide preventative and medical necessity measures to meet their health care needs. Works within the legal and ethical guidelines of the State of Wisconsin as an advanced nurse prescriber.
QUALIFICATIONS: Candidates must have a Master’s Degree in Nursing and Licensed in the State of Wisconsin as Advanced Family Practice Nurse Prescriber. National certification by the American Nurses Credentialing Center as advanced practice nurse practitioner. Current active certification with Drug Enforcement Administration (DEA).

JOB DESCRIPTION: Will coordinate the diabetic education program here at Upland Hills Health. Will work in conjunction with physicians, nursing and other ancillary staff to provide care for all diabetic populations. Duties would include assessment and education of diabetic patients and families.
QUALIFICATIONS: Bachelor's degree and registration with the American Dietitic Association required for a Clinical Dietitian or a Registered Nurse in the State of Wisconsin. At least one full year of work as a Registered Dietitian and/or Registered Nurse and must be a Certified Diabetic Educator or eligible. If a clinical dietitian, may also need to work in other clinical nutrition areas including: long-term care, renal dialysis, outpatients and inpatients.
