| The Joint Commission on Accreditation of Healthcare Organizations will conduct an accreditation survey of Upland Hills Health on Nov. 18 and 19, 2004. The purpose of the survey will be to evaluate the organization's compliance with nationally established Joint Commission standards. The survey results will be used to determine whether, and the conditions under which "Critical Access Hospital" accreditation should be awarded the organization. Joint Commission standards deal with organizational quality of care issues and the safety of the environment in which care is provided. Anyone believing he or she has pertinent and valid information about such matters may request a public information interview with the Joint Commission's field representatives at the time of the survey. Information presented at the interview will be carefully evaluated for relevance to the accreditation process. Requests for interviews must be made in writing and should be sent to the Joint Commission no later than five working days before the survey begins. The request must also indicate the nature of the information to be provided at the interview. Requests should be addressed to: Division of Accreditation Operations, Account Representative, Joint Commission on Accreditation of Healthcare Organizations, One Renaissance Boulevard, Oakbrook Terrace, IL 60181. The Joint Commission will acknowledge requests in writing or by telephone and will inform the organization of the request for any interview. The organization will, in turn, notify the interviewee of the date, time and place of the meeting. This notice is posted in accordance with the Joint Commission's requirements. | |

| Bars of soap, shaving cream, Gatorade and hard candy are just a few of the items being collected for U.S. troops, stationed oversees. This special collection is being coordinated by the Upland Hills Nursing and Rehab Center’s Activities Department. | |
| Items may be dropped off from now until Friday, Nov. 5, at either the NRC or the hospital employee entrance. With questions, contact Toni Rochon at 930-7600. | |
| Items suggested include: bars of soap, powdered laundry soap (Tide without bleach), shaving cream, baby wipes, electrical tape, deodorant, disposable razors, lotion, sunscreen, notebooks, envelopes, magazines, black garbage bags, powdered Gatorade, toilet paper, hard candy, any non-perishable food items (except chocolate, as it would melt.) | |

| Quilters are invited to display their craftsmanship at the 'Quilts in the Cornfield' quilt show and luncheon on Friday and Saturday, Sept. 24 and 25, at the Iowa County Fairgrounds. The event is sponsored by the Upland Hills Auxiliary to benefit patient care, programs and medical equipment at Upland Hills Health. | |

| Quilts, quilted apparel and household items are welcome, including antique quilts, machine quilts, hand quilting, tied quilts, wall hangings and baby quilts. | |
| Quilted items should be dropped off at the Iowa County Fairgrounds, Mineral Point, on Thursday, Sept. 23, from 5 p.m. to 8 p.m., and then picked up at the conclusion of the show. Claim tickets will be provided and required. Quilters are encouraged to write brief descriptions of interest about their quilts on 3"x5" notecards to display near the quilts. | |

| • | Ride & then walk for healthy hearts! |
| Free bus transportation and a fun Saturday outing are being planned for Saturday, Oct. 9. Area residents are invited to take part in the American Heart Association's closest "Heart Walk" at Warner Park, Madison. | |
| Sign up in the Cardiac Rehab Department, ground floor, during normal business hours. Plan to park & ride with the bus. There will be a group stretch-out before the walk, led by Marianne Crowley, RN, Exercise Specialist. The Heart Walk will have oodles of healthy refreshments, and when the walk ends at approximately 10:30 a.m. the Upland Hills team bus will move on to the Dane County Farmers Market. Nutrition Services will offer information on selecting and using fall produce. | |
| Shop the market and eat on the square or State Street area. The bus will be back in Dodgeville by 1:30 p.m. on Saturday. | |
| Walkers are asked to collect pledges to benefit the American Heart Association and its efforts to prevent heart and blood vessel disease.
